Wound Care

Wound care management is the process of treating wounds on an ongoing basis to prevent and/or manage infections by providing an appropriate environment for the wounds to heal. Wound care management is especially critical when patients have acute and chronic wounds. As part of its clinical services, Heart-to-Heart Healthcare offers wound care to its patients at the clinic, and if so required, at the patients’ homes. When providing wound care services, our nurses and caregivers will assist in providing services which are related to cleansing, dressing, treating infections, right up to the healing process

Home Nursing

We do understand the challenges and difficulties that may arise when there is a need to take care of family members who require constant medical attention, for example, elderly parents, the disabled, bedridden patients, and also those who require Alzheimer’s and Dementia care. Based on our personal observations, we realised the difficulties and challenges faced when caring for a family member who falls within one or more of the categories mentioned above. Some family members may not be able to provide proper home care, whilst some others might not be prepared or even have the confidence to do so. Family caregiving may even lead to unnecessary stress, exhaustion and friction among family members.
We believe that by bringing nursing care to your doorstep, you will save time, energy and cost as there will not be any hassle of travelling to the clinics/hospitals, and waiting in line to be attended to due to the long queues. You will also feel completely at ease, as you will receive professional nursing care by trained and certified nurses, who will be able to assist you with your medical needs in the comforts of your home.
Our services include follow-up treatment such as administration of medication, the regular taking of vital signs, wound/dressing care, intravenous infusions, Ryle's tube insertion, change of urine catheters and other medical needs.

Our range of medical solutions include physiotherapy services where we provide specialised treatments to promote the recovery and rehabilitation of injuries, and to maximize the potentials of our patient’s body movements and functions.

Digital Healthcare

In the age of digital healthcare, patients are now more focused towards demanding up-to-date information about their health on a regular basis, in the most efficient manner. Patients now do not want to wait for a whole year before undergoing their annual medical check-ups. Instead, they are looking for an alternative i.e., digital healthcare that can facilitate them when it comes to getting the information and treatment they need.
Taking advantage of the advancements in technology, we continuously work towards providing the best form of digital healthcare to our patients. These include the offering of a state-of-the-art medical device with Wi-Fi/Bluetooth connected to the patient’s mobile phones, that provide accurate information on the readings of the patient’s blood pressure, heart/pulse rate, SpO2, ECG, and temperature. This medical device is also used for glucose monitoring, where instead of the conventional way of pricking the patient’s finger with a needle, the patient now only needs to touch on the device’s sensor and get an update on his/her glucose level.

In-House Medical Consultant

As an in-house medical consultant, we carry out the required pre-employment medical check-up of the organisation’s new employees, the regular check-up of all employees especially those working in hazardous work areas, and we also offer the necessary information on medical issues, specifically the ones related to occupational health.
